Billing
and Insurance
Our office
participates with numerous insurance companies and managed
health care programs. For patients that are members
of one of these plans, our business office will submit a
claim for services rendered. If a patient has
insurance that we do not participate in, our office will
file an insurance claim as a courtesy to you; however,
payment in full is expected at the time of service.
Insurance
may cover all or part of the fee for service; charges not
covered by insurance are your responsibility. Since
mental health benefits may differ from your medical
benefits, please consult your insurance company’s
description of benefits brochure or call your insurance
carrier to determine your benefits, co-pay, and deductible
amounts. If pre-authorization is necessary, be sure
to complete this prior to your appointment.
Professional fees may be paid by cash, check, Visa,
MasterCard, or Discover at the time of the visit.
You will be responsible for your co-payment and deductible
at the time of service for insurances with which we are a
participating provider. It is the patient’s
responsibility to provide us with current insurance
information and to bring their insurance card to each
visit.
We
charge for appointments missed or cancelled without 24
hours advance notice. Charges for missed
appointments are not covered by insurance.
Please
feel free to contact us at (517) 353-3070 with specific
questions related to insurance and fees.
